Sure he did not get a raise!
It is not uncommon in all kinds of industry that seasoned techs do it by the seat of their pants and don’t follow exact procedures every time. Which they should. We probably don’t even want to know.

Please check the facts: it was ONE windscreen that failed.

It happened in 1992 – over 30 yr ago. Did the technology exist to create such a clear and well-focused photo?
Take another look at the 2 photos and re-read the description of how the crew attempted to pull the captain back into the cockpit.
2nd clue:
In the exterior shot, the captain is in a supine position above the open pane with only his legs inside the cockpit via the top of the empty pane.
In the interior shot, he is now in a lateral position resting against the nose of the jet. His legs pass into the cockpit via the side of the pane.
In the narrative, it appeared that the crew was only able to move the captain enough to move his feet off the controls to stop the dive.
The interior shot showed no indication of the loss of pressure pulling loose items toward the open panes (everyone’s hair in place & the coiled cable hanging perfectly vertical)
3rd clue:
Is it possible that for 30 years no one wanted to recreate such a dramatic event for a documentary or a TV movie?
Bless your heart, you need to do a little work on your observational and critical thinking skills.

Unbelievable that this captain did not die. I wonder if any of the maintenance people were fired ?
